特發性血小板減少性紫癜（英語：Idiopathic thrombocytopenic purpura，ITP）是因血小板免疫性破壞，導致外周血中血小板減少的出血性疾病。是最常見的血小板減少性紫癜，它是一種由複雜的多種機制共同參與的獲得性自體免疫性疾病，其特徵是孤立的血小板減少症，而沒有臨床上明顯的情況導致血小板計數低；沒有可靠的實驗室檢測來確認診斷。本病也稱為原發免疫性血小板減少症(英語：primary immune thrombocytopenia)、自體免疫性血小板減少性紫癜（Autoimmune thrombocytopenia，ATP)。

流行病學

本病通常是成年人的慢性病。在美國的一篇綜述中提到，兒童的患病率約為8/10萬人口，成人的發病率約為12/10萬人口^[1]。一些研究也報告了本病的發病率隨年齡增長而增加^[2]，60歲以上的人群發病率為60歲以下人群的2倍。男女發病率相近，育齡期女性發病率高於男性。還有一些研究表明，有的人是在檢查的過程中被確診的，因為他們沒有表現出相關症狀，ITP個體中有1/5至1/3是無症狀的；因此，有症狀表現出來的發生率可能要低得多^[3][4]。

病因與發病機制

本病的病因迄今為止仍尚不明確，發病機制可能為^[5]：

1. 體液免疫和細胞免疫介導的血小板過度破壞
2. 體液免疫和細胞免疫介導的巨核細胞數量和質量的異常，血小板生成不足

根據研究表明，存在着一些誘發本病的因素。諸如：遺傳因素和後天因素。如下^[6][7][8]：

1. 感染：細菌或病毒感染與ITP發病有關。
2. 免疫因素：感染不能直接導致ITP，當免疫功能受損時促進自身反應性抗體的形成，可能是ITP發病的重要原因。
3. 肝、脾作用：肝臟和脾臟可起到破壞血小板的作用。
4. 遺傳因素：在一定程度上可能受基因調控。
5. 其它：雌激素可能與本病的發病有關。

臨床表現

瘀點

許多患者在一般情況下起病隱匿，常表現為乏力，生活品質下降，反覆的皮膚出血如瘀點、紫癜、瘀斑，黏膜出血如鼻出血、牙齦出血，月經過多，及外傷後止血困難等。有的可出現失血性貧血，但是有部分患者雖然存在血小板減少但是並沒出血症狀。內臟出血較少見。肝、脾、淋巴結很少有腫大表現。若患者遭受到感染，病情可驟然加重，可能出現廣泛的、嚴重的皮膚、黏膜及內臟出血。^[5]

實驗室檢查

根據一些指南^{[9][10][11][12][13]}對懷疑患有本病的患者可以進行的實驗室檢查如下：

• 外周血塗片：以確認血小板減少不是由於血小板結塊（即假性血小板減少症）造成的人為因素，並且沒有諸如缺乏等形態性血小板異常血小板顆粒或均勻大小的血小板，可能提示遺傳性血小板疾病。
• 血常規：血小板計數減少（血小板計數<100,000 /μL），血小板平均體積偏大。
• 出凝血及血小板功能檢查：凝血功能正常，出血時間延長，血塊收縮不良，束臂實驗陽性。血小板功能正常。
• 骨髓象檢查：骨髓巨核細胞數量正常或增加，巨核細胞發育成熟障礙（表現為體積減小，胞內顆粒減少，幼稚巨核細胞增加）；有血小板形成的巨核細胞顯著減少，紅系、粒系及單核系正常。
• 免疫學檢查：80%的患者血小板相關抗體（PAIg）、血小板相關補體（PAC3）兩者呈陽性。
• HIV和HCV測試：對所有患者進行HIV和丙型肝炎病毒（HCV）感染測試，因為血小板減少症是這些情況的常見表現，治療潛在的感染可能會提高血小板計數^[12][13]。

診斷與鑑別診斷

本病主要為排除診斷。其重要組成部分包括排除血小板減少症的其他可能原因以及確定可能導致繼發性ITP的疾病。應從病史，體格檢查和實驗室檢查三個方向進行判斷，若未顯示出血小板減少症的其他潛在病因時，可以做出原發性ITP的推定診斷。重要的是，對於輕度血小板減少症且無臨床出血的患者，ITP的診斷並不意味着需要治療。

本病需要排除假性血小板減少症及繼發性血小板減少症，如再生障礙性貧血、遺傳性血小板減少症、肝病和（或）脾功能亢進、骨髓增生異常綜合症(MDS)、白血病、系統性紅斑狼瘡、藥物性免疫性血小板減少症、微血管病變過程等。

治療

本病屬於自身免疫性疾病，目前尚無根治的方法，治療目的是讓患者血小板計數升高到安全水平而不是正常化^[9][14][15]，降低病死率。

一般治療

出血嚴重者應注意休息，血小板<20×10⁹ /L者應嚴格臥床，避免外傷，並服用止血藥及局部止血。若患者無明顯的出血傾向，血小板計數>30×10⁹ /L，無手術、創傷，且不從事增加出血危險的工作或活動，發生出血的風險小的，可以觀察隨訪，一般不需治療。

一線治療

糖皮質激素

糖皮質激素為ITP的首選治療，近期有效率達80%。其作用機制為：減少PAIg生成及減輕抗原抗體反應，抑制單核-巨噬細胞系統對血小板的破壞，改善毛細血管通透性，刺激骨髓造血及血小板對外周血的釋放

常用藥物有潑尼松、地塞米松、甲潑尼龍等。

靜脈輸注丙種球蛋白(IVIg)

主要用於ITP的緊急治療，不能耐受糖皮質激素治療的患者，脾切除術前準備，妊娠或分娩前。

作用機制與封閉單核-巨噬細胞系統的Fc受體、抗體中和及免疫調節有關。注意，IgA缺乏、糖尿病和腎功能不全者慎用。

二線治療

對於一線治療無效或需要較大劑量糖皮質激素才能維持的患者，可以選擇二線治療。

手術、介入治療

若經正規的糖皮質激素治療無效或不能使用糖皮質激素，可考慮脾切除術治療，有效率約70～90%。但由於術中易出血使手術風險增大。

或者採用介入治療，以脾動脈栓塞造成部分脾梗死也可達到相似的結果。

免疫抑制劑

若經糖皮質激素治療及手術治療仍療效不佳或不宜採用這兩種治療方法時可使用免疫抑制劑，也可以與糖皮質激素聯合使用。

常用藥物有長春新鹼、環磷酰胺、硫唑嘌呤、環孢素等。

其它

• 達那唑，為合成雄激素。
• 氨肽素
• 清除幽門螺桿菌：日本和意大利研究者發現幽門螺桿菌的感染與ITP有關，部分患者經抗幽門螺桿菌治療後血小板計數升高。^[16][17]

急症處理

適用於伴消化系統、泌尿生殖系統、中樞神經系統或其他部位的活動性出血或需要急診手術的重症ITP患者^[18]

• 血小板輸注
• 靜脈輸注丙種球蛋白IVIg)
• 促血小板生成藥
• 大劑量甲潑尼龍
• 重組人活化因子Ⅶ(rhFⅦa)
