長胸神經（nervus thoracicus longus），又稱「外呼吸貝爾神經」（external respiratory nerve of Bell）^[1]，源自於第五至第七對頸脊神經（英語：cervical nerves）（C5-7）的脊神經前支（英語：anterior rami），主要支配前鋸肌。一般來說C5和C6會穿過中斜角肌（英語：scalenus medius），C7則會從該肌肉前方穿過，有時C7不會加入長胸神經。

結構

胸長神經起自C5-C7神經根，在臂叢主要結構後向外下進入腋窩，沿胸側壁前鋸肌表面隨胸外側動脈下行。^[2]^[3]

變異

在大約 10% 的人群中，胸長神經也接受來自 C8 的神經供應。^[3]

臨床意義

翼狀肩

翼狀肩是指胸長神經損傷引起前鋸肌癱瘓，肩胛骨脊柱緣翹起呈翼狀。^[4]乳房切除術、臂叢炎可能造成胸長神經的損傷，從而導致翼狀肩。^[5]

可用胸背神經的後支支配胸長神經以治療翼狀肩。^[5]^[6]
