里急後重（英語：rectal tenesmus）是一個醫學術語，形容便秘時的一種症狀。患者感覺急需大便而無法順利排出；可用16個字概括：「腹痛窘迫，時時欲便。肛門重墜，便出不爽」。[1]
中文病名出自中國古代醫學著作《難經·五十七難「疾病」》。便前腹痛，急欲大便，叫「里急」。大便時急迫欲便，而有便不盡之感，叫「後重」。[2]
