偏执狂（paranoia，台湾又译妄想狂）在医学中是一种具有极度焦虑及恐惧特性的思考方式。且其经常是非理性的妄想。“Paranoia”一词源自希腊文的“παράνοια”，大致的意思是“疯狂”。过去这个字也用来表示各种妄想状态。

偏执狂与恐惧症的差别在于恐惧症对于各种事物的非理性及永久性恐惧，通常是没有理由的。而偏执狂通常是害怕他人有目的的行为。
