北魏建義元年（528年）置永安郡，屬晉州，治所在永安縣（今山西霍州市）。轄境相當今山西霍州市及洪洞縣地。東魏、北齊沿置，北周下轄永安縣、楊縣。隋文帝開皇三年（583年）廢永安郡，永安縣後來改為霍邑縣、楊縣改為洪洞縣。
