史玉泉（1919年6月18日—2022年3月17日），男，浙江餘姚人（一作上海人），中國神經外科專家，中國神經外科的主要創建人之一。[1]曾任上海醫科大學神經病學研究所所長、教授、博士生導師。[2][3][4]
1938年，考入國立上海醫學院。抗日戰爭結束後，從重慶市民醫院調至上海中山醫院工作。[1][5]1950年12月20日，史玉泉作為沈克非的助手，成功施行中國第一例腦瘤手術。[6]1952年，調到上海第一醫學院附屬內科學院（復旦大學附屬華山醫院的前身），並與朱禎卿創建中國第二個神經外科。[6]1956年7月，加入中國共產黨。[7]1989年6月退休。[5]
2022年3月17日病逝，享年102歲。[8]
