镫骨（stapes /ˈsteɪpiːz/、stirrup）是中耳的一块听小骨，存在于人类和其他哺乳动物。这块骨头形如马镫，贴在卵圆窗上，负责将声波震动由此处传送至内耳。镫骨是人体最小且最轻的骨头，其英文名称“stapes”即为拉丁文的马镫。

外形

镫骨是中耳三块听小骨中的最后一块，以环形韧带和卵圆窗相连。它状似马镫，足板（即底部）座落于卵圆窗，头部和砧骨形成关节，头部和足板间以前脚和后脚连结^[1]^:862。镫骨和砧骨间则为砧骨镫骨关节（英语：incudostapedial joint）^[2]。镫骨是人体最小的骨头，大小约3 x 2.5公釐，头部到足板的轴较长^[3]。

发育

镫骨发育自第二咽弓（英语：pharyngeal arch），在胚胎发育第6－8周逐渐成形。胚胎时期人体具有镫骨血管（英语：stapedial artery），为耳后动脉的分支，但之后会退化，因而形成镫骨中央的孔洞^[2]^[4]。

变异

相较于其他骨头，不同种族的人们镫骨尺寸差异较小^[5]。约0.01－0.02%的人镫骨血管没有退化，而持续留存于中央孔洞^[6]，他们有可能因此听到脉搏，但也可能完全没有症状^[7]。有些人则完全没有镫骨，但这种情况非常罕见^[8]^[9]^:262。

其他动物

镫骨是哺乳动物三块听小骨的最后一块。在非哺乳类四足动物，和镫骨同源的骨头是它们唯一的听小骨，称为耳柱骨；然而也有人用镫骨来描述爬虫类的听小骨。两生类的耳器具有五块软骨，镫骨和其足板分别对应到中拨部（pars media plectra）和内拨部（pars interna plectra）^{[注 1]}；镫骨和其同源构造最初由鱼类的舌颌软骨（英语：hyomandibula）演化而来，属于鳃弓（英语：gill arch）的一部份，用于支持气门或下颌（依种类不同）。^[2]^[11]^:481–482

镫骨位处砧骨和内耳之间，负责将来自砧骨的震动传到卵圆窗；卵圆窗是通往内耳的通道，有一层膜分隔内耳和中耳。由于镫骨足板和卵圆窗的面积比鼓膜小很多，因此能增加中耳对内耳的压力，克服卵圆窗传递声波时面临的阻抗。^[12]^:289

镫骨肌附著在镫骨颈上，负责维持镫骨的稳定，由颜面神经支配。镫骨肌收缩将使镫骨较不易震动，从而防止过强的声音对内耳造成伤害。^[1]^:861–863

耳硬化症

耳硬化症（英语：Otosclerosis）是一种先天性或自发性的疾病，患者内耳发生异常的骨质重塑，造成镫骨黏著在卵圆窗上，丧失传递声音的能力，导致患者传导性耳聋。临床上约有1%的人患有耳硬化症，但在多数案例中不会产生可察觉的症状。青少年和妇女有较高的耳硬化症发生率^[13]。目前主要的治疗方法有两种，镫骨切除术（英语：stapedectomy）将镫骨移除后换上人造镫骨；镫骨足板造孔术（英语：stapedotomy）则在镫骨足板（即底部）打一个小洞，然后将义肢插入洞中^[14] ^:661。若患者的镫骨血管没有退化，手术会变得更复杂；镫骨足板纤维化或阻塞性耳硬化等因素也会使手术难度增高^[6]^[9] ^:254–262。目前普遍将先天性中耳缺陷分为四型，和镫骨黏著相关的第一型和第二型就占了超过一半^[15]。

砧骨镫骨关节不连续

砧骨镫骨关节（英语：Incudostapedial joint）不连续（Incudostapedial discontinuity）是指砧骨和镫骨没有接触，导致震动无法传递，但此症状通常是因为砧骨长突毁损或骨质吸收造成。手术可使用自体移植的骨头或义肢来传递震动，绕过锤骨砧骨关节，使锤骨的震动能直接带动镫骨；在较轻微的患者中也可以打入骨水泥，让听小骨间重新接触。^[16]

一般认为镫骨是拿坡里大学的乔望尼·英格拉西亚（英语：Giovanni Filippo Ingrassia）教授于1546年所发现^[17]，相关描述见于英格拉西亚的《解剖纪事》^{[注 2]}，这本书在他逝世后才于1603年出版；但西班牙解剖学家佩德罗·希梅诺（Pedro Jimeno）于1549年发表的《医学对谈》^{[注 3]}中也描述了镫骨，因此最先发现镫骨的人是谁仍存有一些争议^[18]。镫骨因状似马镫而以此为名，其英语名称“stapes”即为拉丁语的马镫，由于早期使用拉丁语的地区还没有马镫，该字可能是中世纪才出现的晚期拉丁语词汇，来自古典拉丁语的“站（stapia）”^[19]。
