อาการระคายเคืองเยื่อหุ้มสมอง หรืออาการคอแข็งเกร็ง[1] (อังกฤษ: meningism) เป็นอาการแสดงทางการแพทย์ประกอบด้วยลักษณะร่วมสามอย่าง ได้แก่ คอแข็ง (nuchal rigidity, neck stiffness) แพ้แสงจ้า (photophobia) และปวดศีรษะ เป็นสัญญาณแสดงว่ามีการระคายเคืองของเยื่อหุ้มสมอง อาจพบในโรคเยื่อหุ้มสมองอักเสบ ภาวะเลือดออกใต้เยื่อหุ้มสมองชั้นกลาง และโรคอื่นๆ
