Isotopomer atau isomer isotop ialah isomer yang mempunyai bilangan yang sama untuk setiap atom isotop, tetapi posisinya berlainan. Umpamanya, CHCHDCH dan CHCHCHD merupakan sepasang istopomer. Isotopomer harus tidak dikelirukan dengan isotopolog.
