Pilsēta nosaukta Polockas Rurikoviču dinastijas pamatlicēja Izjaslava Vladimiroviča vārdā, un esot dibināta 985. gadā, kad Kijivas lielkņazs Vladimirs Svjatoslavičs atsūtīja uz dzimtajām vietām savu sievu Rognedu. No 10. gadsimta - Izjaslavas kņazistes centrs. Mūsdienās saglabājušās hronikās pilsēta pirmoreiz minēta 1127. gadā. 1159. gadā smagi nopostīta kņazu savstarpējo cīņu laikā, un gandrīz uz diviem gadsimtiem pazūd no hroniku ziņām.
No 14. gadsimta atradās Lietuvas lielkņazistē kā Zaslava. Reformācijas laikā Zaslavje bija kalvinisma un sociānistu centrs. Šeit darbojās Bībeles tulkotājs baltkrieviski un poliski Simons Budnijs (Сымон Будны/Szymon Budny). Pilsēta pievienota Krievijai pēc Polijas Otrās dalīšanas 1793. gadā. No 1920. gada - Baltkrievijas PSR sastāvā. Otrā pasaules kara laikā no 1941. gada jūnija līdz 1944. gada 4. jūlijam pilsēta atradās vācu okupācijā. Kopš 1991. gada - neatkarīgās Baltkrievijas sastāvā.
Daļa no pilsētas teritorijas ir arheoloģijas rezervāts. Uz ziemeļiem no pilsētas, Lašanu ciemā, atrodas militārais muzejs "Staļina līnija".
