Karloffin läpimurto oli ohjaaja James Whalen vuonna 1931 valmistunut elokuva Frankenstein, jossa hän esiintyi Frankensteinin hirviönä. Suuri osa muistakin Karloffin roolihahmoista oli hirviöitä tai muita pahoja hahmoja lukuisissa eri kauhuelokuvissa, joista mainittakoon Muumion rooli elokuvassa The Mummy.
